 Health and Medication Monitoring - All aspects of residents care is managed by a Licensed Nurse. Individualized Service Plans are developed with the resident and family/ guardian and Community Support if assigned, to ensure the needs are being met for your loved one. Regular weekly monitoring visits by the Licensed Nurse, medical care including yearly physicals, sick visits and follow-up assistance with ongoing medication for controlled conditions is provided. Significant events (illness, behavioral changes, achievements, etc.) for each resident will be recorded. We notify the residents physician, guardian / family and placing agency of any significant changes in a resident’s condition. We also work with other agencies and organizations involved in the provision of services to our residents. Building relationships with the family and physicians is our goal to providing quality care. All record keeping is completed with state of the art computerized programs, designed for holistc care provision.
